On September 09, 2019, an employee was installing window wrap from a 40-foot extension ladder when he fell about 15 feet to the ground. He suffered fractures to his right shoulder blade, ring finger, and lower back and a laceration to the right side of his face. He was hospitalized. The ladder's feet were not properly positioned at the time of the incident.
